How To Fill Out A Passport Application

One of the most stressful parts of the passport application process is simply going through all of the paperwork. This step-by-step guide will help you fill out an application for a new passport.

Step 1- Gather the Required Documents

You can’t apply for a US passport without documents that prove you are who you say you are and that you are a US citizen. Here are the documents you will need:

Proof of Identity:

A U.S. Passport (can be expired or current); a U.S. passport card; driver’s license; Certificate of Naturalization, Certificate of Citizenship; military identification; or a federal, state, or municipal government employee ID card. Also, make a clear photocopy of your ID to submit with your application.

Proof of Citizenship:

If you were born in the US, a previously issued passport or a certified birth certificate with a raised seal will suffice. If you are a naturalized citizen, you may submit your naturalization certificate or Certificate of Citizenship. If you were born abroad to US citizen parents, you’ll need your foreign birth certificate or a Consular Report of Birth, plus other additional documentation of your parents’ US citizenship and US residency. Read page 2 of the application instructions for details on what is required.

Step 2- Gather the Required Information

To complete the US passport application, you need to know your parent’s full names and where they were born. You also need to gather documentation to support any name changes if your current name differs from the document you are using as proof of citizenship. .

Step 3: Complete the Application

Complete the application in your most perfect penmanship, using a pen with black ink. If the Department of State is unable to read your handwriting, you application could be delayed or declined.

If you have a computer with a printer, you may also choose to complete the application online and print it. This is the best option, as it will prevent your application from being rejected if your handwriting is less than perfect.

You should fill out the application completely, but DO NOT sign it at this time.

Step 4: Go to a Passport Acceptance Facility To Submit Your Application

Next, use the US Department of State’s website to look up the nearest passport acceptance office. These are usually located in places like post offices and libraries. Take your application along with all the supporting documents you have gathered and 2 passport photos. The acceptance agent will make you swear an oath stating that the information on your application is correct and you are eligible to possess a US passport. Then, you will sign the passport application, pay the requested fees and the acceptance agent will send your paperwork to the Department of State for processing.
